Making Appointments

To schedule an appointment, call our dental office at 812-473-1400 and our staff will assist you in finding a day and time that is available and convenient for you.

New Patient Paperwork for Your First Visit

Prior to your first appointment, we ask that you complete our new patient paperwork. To allow you ample time to be as thorough as possible, we recommend downloading the form from the link below and completing these forms at home or arriving at least 20 minutes early for your appointment to complete them in our office.

Brushing and flossing before the appointment will help our dentist properly examine your teeth with efficiency and ease.

Comprehensive Exam

After reviewing your paperwork, paying close attention to your dental and medical history, our dentist will proceed with your oral examination. Our dentist will carefully examine your teeth, gums and other oral tissues, looking for signs of decay, disease and other potential problems.

X-Rays

The dentist may determine that x-rays are necessary. We use digital x-rays at Evansville Family Dentistry, so the process takes just a couple minutes to complete and the images are available immediately for our dentist to evaluate. Cleaning

If no problems were discovered, our dentist recommends patients return to our Evansville, Indiana, office every six months for another checkup and cleaning.
